On Your First Visit
We want your first visit to be pleasant and productive, so please come prepared:
- Please arrive 15 minutes before your scheduled appointment to complete the paperwork.
- Plan to be here for at least an hour.
- Bring:
- Your most current insurance card
- Copy of any other insurance information
- Doctor's prescription for therapy (or call and make an appointment with our Rehabilitation Doctor)
- Our Medical staff will:
- Verify your insurance coverage
- Your physical therapist will:
- Collect detailed medical history.
- Evaluate possible therapies.
- Consult with you and your doctor about a treatment plan.
- You may be able to begin treatment at your first visit.
